Gift Card Terms and Conditions

These terms and conditions govern the purchase, use, and redemption of gift cards issued by Elevate Medical Spa PLLC. By purchasing or using a gift card, you agree to comply with these terms.

Terms of Use

1. Redemption: Gift cards may be redeemed for eligible products or services at Elevate Medical Spa PLLC’s pysical location, subject to availability.

2. Stackable with Other Discounts: Gift cards may be used in conjunction with other discounts, promotions, or offers unless otherwise specified.

3. Purchase Date Restriction: Gift cards cannot be used on the day of purchase.

4. Membership Program Exclusion: Gift cards may not be used to pay for membership programs or subscription services.

5. Expiration: Gift cards expire one year from the date of purchase. No refunds or replacements will be provided for expired gift cards.

6. Free Bonus Offer Expiration: Promotional value must be used prior to 4/1/2026 e.g. the promotional $20 free amount with $200 gift card purchase expires 4/1/2026. The $200 gift card amount expires after 1 year.

7. Non-Transferable: Gift cards are non-transferable and may not be resold or redeemed for cash, except where required by law.

8. No Cash Value: Gift cards cannot be exchanged for cash, except as required by applicable law.

9. Lost or Stolen Gift Cards: Elevate Medical Spa PLLC is not responsible for lost, stolen, or damaged gift cards. Treat gift cards like cash.

10. Fraudulent Use: Gift cards obtained through fraudulent means are void. Elevate Medical Spa PLLC reserves the right to refuse redemption of any gift card suspected of being fraudulently obtained or used.

12. Disputes: In the event of a dispute, the decision of Elevate Medical Spa PLLC is final.

At our clinic, we are committed to supporting your health goals in a way that is structured, flexible, and ethical. Prepaid packages and memberships are designed to honor your commitment to care while offering a modest discount for those undertaking regular treatment.
general guidelines
All packages and prepaid bundles require an initial consultation to determine your clinical needs and suitability for a structured course of care. Packages cannot be purchased before your initial consultation. New clients must book an Initial Consult before accessing any treatment packages. Treatment plans are individually tailored following your first consultation, at which point a package may be recommended based on your condition and goals. Please note: not all conditions require multiple treatments. The frequency and duration of care will be based on your individual presentation and your goals, in line with best practice and ethical standards
package details & conditions
Payment: All packages must be paid in full at the time of purchase. Packages and prepaid bundles expire 12 months from the purchase date. Sessions not used within this period will lapse. Booking: Appointments are scheduled on a ‘first come, first served’ basis. Every effort will be made to accommodate your needs. Non-transferable: Packages are not transferable between patients.
refunds & changes
Refunds: If you experience significant personal hardship or exceptional circumstances, unused sessions may be refunded on a pro-rata basis. The refund will be calculated by charging full standard rates for completed sessions and returning the unused balance. For example: A 5-treatment package priced at $625 (with standard value of $650) — if you use 2 sessions, the remaining refund would be based on subtracting 2 x $125 = $250 from the package total. The refund would be $375. Policy change notice: We reserve the right to change or end any promotional pricing or package offering at any time without prior notice.
cancellations & no-shows
The clinic’s standard cancellation policy applies to all package and membership bookings. Missed appointments or late cancellations may incur a cancellation fee which is payable separately and not deducted from your prepaid package.
Exclusions
Once your prepaid package is complete or expired, standard rates apply for any future treatments, unless a new package or membership is purchased.
